Permissions *

I give permission for SuPaw Nanny to walk my dog with other dogs from different households as well as the resident dogs.

I would like my dog(s) walked on a lead at all times or;
I give my permission for SuPaw Nanny to walk my dog(s) off lead in suitably enclosed areas and fields away from roads. In doing so, I confirm that I am confident that my pet(s) can be walked and socialised with other animals, and is friendly towards them and other people. NB: Neither SuPaw Nanny nor its staff will be held financially responsible for any damage to property, and/or injury to other people/animals, caused by client's pets during the period in which they are in her care.

I consent to my dog(s) at any time being transported in a secure crate in SuPaw Nanny's purpose-built vehicle.

Specific Walking Instructions

Dogs in our care will receive a minimum of one walks each day, for a minimum of 60 minutes subject to life stage, physical and mental health, veterinary advice and breed of each dog. Should your dog(s) have any specific exercise requirements contrary to this, please state them here and we will exercise your pet(s) pursuant to your specific instruction and consent as set out above. NB: We only let dogs off lead after a period of getting to know them and in the fields away from the main road. We can use long lines as an extra precaution (if requested above). We will only let your dog off lead if/when we feel comfortable and confident to do so. So, if this is your dog’s first stay with us or they have not been for a while, we will work up to them being off lead if you have ticked the above 'off-lead' permission.

Medical Permissions *

I understand that my dog(s) will not be offered any walks unless I am able to provide evidence that they have received the required vaccinations against Parvovirus, Distemper, Hepatitis, Leptospirosis and Kennel Cough at least two weeks prior to walking out with SuPaw Nanny.

I understand that my dog(s) will not be walked if they have had (within 21 days prior or currently have, an infectious disease).

In the event of a clinical/medical emergency where I am unavailable or cannot be reached, I give full consent to SuPaw Nanny to make decisions regarding my pet(s) health provided it is at all-times acting in the best interests of my pet and on the advice of a veterinary surgeon. And as owner, I am responsible for payment of any veterinary fees incurred by this and will be required to refund SuPaw Nanny any expenses from seeking medical attention within seven days. Including transportation (mileage will be charged at 0.50p/mile).

I authorise SuPaw Nanny to utilise her own Registered Veterinarian in the event that my pets regular veterinarian is unavailable. As such, I agree to reimburse SuPaw Nanny for any additional fees for providing emergency care, as well as any expenses incurred for unexpected visits, transportation (to include mileage which is charged at 0.50p/mile) within seven days.

Vaccination Renewal *

Please state the renewal date of your pet(s) vaccination. An up-to-date veterinary vaccination record must be seen prior to walking to ensure that dogs have current vaccinations against Parvovirus, Distemper, Hepatitis, Leptospirosis and Kennel Cough. Certification from a veterinarian of a recent protective titre test may be accepted instead of a booster vaccination. The certificate must state that it is valid for the current period of walking out with SuPaw Nanny. Primary vaccination courses must be completed at least two weeks before walking out with SuPaw Nanny.

Flea, Tick, Lice, Worm Treatment Renewal *

Please state the renewal date of your pet(s) Flea, Tick, Lice and Worm treatment. Your dog(s) must have been appropriately treated for internal and external parasites in accordance with veterinary advice before walking out with SuPaw Nanny. If there is evidence of fleas, ticks, lice or worms your dog will be treated with an appropriate product authorised by VMD & SuPaw Nanny will inform you, the owner, as and when parasites are discovered and invoice you for the cost.
